Top 10 Tips for Passing the MRCEM Primary Examination
The MRCEM Primary is the first written component of the Membership of the Royal College of Emergency Medicine examination. It consists of 180 single best answer (SBA) questions completed over three hours and is mapped to the RCEM Basic Sciences Curriculum and the wider RCEM Curriculum.
The syllabus is broad. Candidates are expected to understand the scientific principles that underpin Emergency Medicine, including anatomy, physiology, pharmacology, microbiology, pathology, biochemistry and evidence-based medicine. Success therefore depends on much more than memorising isolated facts. You need to understand the core science, recognise how it is applied in clinical situations and answer accurately at a consistent pace.
We have drawn on our experience supporting MRCEM candidates, patterns identified across many years of examinations and the practical advice that repeatedly helps doctors prepare effectively. Everyone will approach revision slightly differently, but these ten principles provide a reliable framework.
1. Start With the Curriculum
Before opening a textbook or beginning a question bank, become familiar with the RCEM Basic Sciences Curriculum.
The curriculum defines the knowledge that may be assessed and should form the structure of your revision. Without it, it is easy to spend too much time on interesting but low-yield material while missing smaller areas that can still contribute several questions to the examination.
Break the curriculum down into subjects and then into manageable topics. Use it as a checklist throughout your preparation, marking areas as you cover them and returning regularly to anything that remains weak.
No revision resource should replace the curriculum. Textbooks, tutorials, notes and question banks are most useful when they help you understand and test the material within it.
2. Give Anatomy and Physiology the Time They Deserve
Anatomy and physiology make up a substantial part of the MRCEM Primary and should sit at the centre of your preparation.
For anatomy, concentrate on clinically relevant relationships rather than simply learning lists of structures. You may need to predict the consequences of a nerve injury, identify what could be damaged by a fracture or understand how infection and blood can spread through anatomical spaces. Neuroanatomy, limb innervation, the brachial and lumbosacral plexuses, abdominal anatomy and the major vessels all deserve careful revision.
Physiology is similarly broad. Cardiovascular, respiratory, renal and gastrointestinal physiology are particularly important, but no major system should be neglected. Aim to understand the mechanisms involved. If a physiological variable changes, you should be able to work through the likely effects rather than relying entirely on remembered statements.
These subjects take time to learn properly, so begin them early and revisit them repeatedly.
3. Understand the Science Rather Than Memorising Lists
There is inevitably a significant amount of factual knowledge to remember for the Primary. However, factual recall alone is rarely enough.
Questions often place a basic science principle within a short clinical scenario. A patient may have an injury affecting a named nerve, a drug may alter a physiological response or a disease process may produce a predictable biochemical abnormality. The underlying knowledge is still basic science, but you must recognise which principle is being tested and apply it correctly.
When revising a fact, ask yourself why it is true and how it might become relevant in the Emergency Department. If you are learning a drug, understand its mechanism, important effects and adverse effects. If you are revising a nerve, consider its course, motor and sensory functions, and the findings produced by injury.
Understanding makes unfamiliar questions easier and also makes the knowledge far more likely to remain available under examination pressure.
4. Begin Practice Questions Early
Do not wait until you feel that you have finished learning the syllabus before starting questions. With a curriculum this broad, that point may never arrive.
Question practice should guide your revision from an early stage. It helps you identify genuine gaps, become familiar with the level of detail expected and recognise how straightforward scientific knowledge is turned into an SBA.
Begin with questions grouped by topic while you build your knowledge. After attempting a set, use the explanations to direct further reading and then return to the subject later. As your preparation progresses, introduce mixed question sets so that you have to identify the topic being tested without being prompted by the name of the section.
Questions are not simply a way to measure what you know. Used properly, they are one of the most effective ways to learn.
5. Learn From Every Incorrect Answer
The questions you answer incorrectly often provide the most valuable part of your revision.
Do not simply note the correct answer and move on. Ask:
- What piece of knowledge was I missing?
- Did I misunderstand the underlying principle?
- Was there a clue in the stem that I overlooked?
- Why were the other options less appropriate?
- Would I answer a similar question correctly if it were worded differently?
Keep brief notes of important errors, repeated weaknesses and facts that you regularly confuse. These notes should be selective. Copying every explanation creates another large resource that you will never have time to revise.
Revisit incorrect and flagged questions after a suitable interval. The aim is to confirm that you have corrected the underlying weakness, not merely remembered the answer from your previous attempt.
6. Do Not Neglect the Smaller Subjects
Candidates naturally devote most of their time to anatomy and physiology, but the remaining subjects collectively represent a significant part of the examination.
Pharmacology, microbiology, pathology, statistics and evidence-based medicine can all provide valuable marks. Many of these areas are relatively contained and respond well to structured revision. Once the central principles and common patterns are understood, performance can improve quickly.
Pay particular attention to topics that are easy to postpone, such as antimicrobial mechanisms, patterns of inheritance, hypersensitivity reactions, study design, diagnostic test characteristics and the interpretation of common statistical measures.
You do not need the depth expected of a specialist in each discipline. You do need dependable knowledge across the whole curriculum. A few avoidable gaps spread across several smaller subjects can make the difference between passing and narrowly missing the required standard.
7. Build Concise Revision Notes
By the final few weeks, you should ideally have a manageable set of personal notes that reflects your own weaknesses.
These notes do not need to reproduce the curriculum. Record information that you repeatedly forget, concepts that took time to understand, useful comparisons and common points of confusion. Tables, short diagrams and carefully chosen mnemonics can be particularly helpful for information that is difficult to organise.
Keep refining your notes as your knowledge improves. Remove material that has become secure and add new points only when they are genuinely worth revisiting.
The result should be a concise resource that you can review quickly near the examination. A short set of notes built from your own question practice is usually more valuable at that stage than returning to several large textbooks.
8. Practise Mixed Questions and Timed Mocks
Topic-based practice is useful early in revision, but it does not fully reproduce the demands of the examination. In the MRCEM Primary, an anatomy question may be followed by pharmacology, statistics or renal physiology. You must repeatedly change subject and identify what each question is testing.
Introduce mixed blocks once you have established a reasonable foundation. Later, complete full-length mock examinations under realistic conditions.
The examination allows approximately one minute per question. That is usually sufficient, but only if you maintain a steady pace and avoid spending too long on a difficult item. Timed mocks help you develop that pace and reveal whether concentration falls during the later part of the paper.
They also give you an opportunity to refine your examination technique. Read the stem carefully, identify what is actually being asked and choose the best answer available. If you are uncertain, make a reasoned choice, flag the question if the system permits and return to it after completing the rest of the paper.
9. Use Repetition Intelligently
The volume of material in the Primary makes repeated exposure essential. One pass through a subject is rarely enough.
Repetition should be spaced and purposeful. Return more frequently to weak or easily forgotten topics while allowing longer intervals between subjects that are already secure. Reattempting questions can be helpful, provided that you are recalling the principle rather than recognising the wording.
Try to explain difficult concepts without looking at your notes. Draw an anatomical pathway from memory, describe the sequence of a physiological response or compare two easily confused drugs. This type of active recall is much more effective than repeatedly rereading the same page.
Your revision should gradually shift from broad learning towards targeted consolidation. By the final phase, most of your time should be spent retrieving knowledge, correcting persistent weaknesses and practising its application.
10. Make Your Revision Plan Realistic
Most candidates prepare for the MRCEM Primary while working a demanding rota. A plan that depends on several uninterrupted hours every day is unlikely to survive contact with nights, late finishes and normal life.
Build your schedule around consistent, achievable sessions. Short periods of focused study can be used for questions, reviewing notes or revisiting a small curriculum area. Reserve longer sessions for difficult topics and mock examinations.
A useful structure is:
- Early phase: establish the core anatomy and physiology while beginning topic-based questions
- Middle phase: complete the remaining curriculum and increase mixed question practice
- Final phase: concentrate on timed mocks, weak areas and concise revision notes
Include some flexibility. Missed sessions should not cause the entire plan to collapse. Protect sleep, continue exercising where possible and take proper breaks. Sustainable preparation over several months is more effective than an exhausting final burst that leaves you tired before the examination begins.
Final Thoughts
The MRCEM Primary is a demanding examination, but its breadth becomes manageable when your preparation has a clear structure.
Start with the curriculum. Give anatomy and physiology sufficient time, but maintain coverage of every subject. Use questions from the beginning, learn carefully from mistakes and move towards mixed, timed practice as the examination approaches.
Most importantly, aim to understand the scientific principles rather than memorising isolated answers. This gives you the flexibility to deal with unfamiliar questions and provides a stronger foundation for the later MRCEM examinations and your clinical practice in Emergency Medicine.
